Students are required by the EEC to have a current physical on file. Physicals are valid for one year and must be signed by your child's physician. If medication is required for your child, it must be in the original container, and it must be accompanied by a written permission note with directions and signed by you and your doctor in order for us to dispense this medication to your child. All students must have a signed medical release form on file before we can dispense any medication. These forms are available upon request.

Day Care students are required to have a signed doctor's Tylenol Release Consent Form (in case of fever). Note: An attempt to reach the parents will always precede any dispensing of Tylenol to students.

We strongly suggest that parents use good judgment when determining whether a child is sick. Please remember that when your child is sick, they belong at home. It is not fair to your child or others to send your child to school when he/she is not feeling well, especially if they have not attended public school that day. Students who have vomited or had diarrhea in the past 12 hours should not be at Busy Bee.

The following requires a doctor's note to return to school: 
* Conjunctivitis * Strep Throat * Chicken pox * Lice * Mumps * Fifth Disease * Measles * Impetigo 

Procedure for Mildly Ill Students:
If a child is mildly ill, he/she is encouraged to lie on a mat with a pillow and blanket . If feeling up to it, he/she can take part in daily activities. Parents are notified and asked to make the decision on whether or not to dismiss the child from school. 

Children With Disabilities 

7.10 (2) 
accept applications or any child with a disability - with parental consent information requested from:
Local Education Agency, Early Intervention, Health / Service providers
(a) with parental input identify in writing
accommodations
1. change/modification in child's participation
2. size of group to which child is assigned and staff/child ration
3. Any special __ equipment ___ materials ____ ramps ____ aids 

(b) written notification to parent within thirty days of receipt of information if accommodations cause undue burden includes:
___ reasons for decision
___ may contact office for determination of compliance
___ maintain copy of notification in records 

(c) in making determination, consider
1. nature and cost 
2. ability to secure funding
3. licensee financial resources
4. # of persons employed
5. effect on expenses/resources, other impact

Toileting needs are not undue burden.

(d) with parental permission 
___ participate in development /review of child's program plan with LEA, EI, other health and service providers.

(e) with parental permission 
___ inform administrator of special ed. in writing re: serving a child with special needs.
